Long-Term Care facility providing hands-on clinical placement experience.

• Provided personal care to 10–15 residents per shift, including bathing, dressing, grooming, toileting, oral hygiene, and feeding, following each resident's individualised care plan.
• Completed safe transfers using mechanical (Hoyer) lifts, sit-to-stand lifts, wheelchairs, gait belts, and two-person assists under supervision.
• Documented resident care, food and fluid intake, output, and behaviour changes and reported concerns promptly to the charge RPN.
• Supported 6–8 residents living with dementia using Gentle Persuasive Approaches (GPA) and person-centred care to promote comfort and reduce responsive behaviours.
• Assisted residents with feeding under dysphagia precautions, following diet orders and aspiration-prevention techniques.
• Maintained infection prevention and control (IPAC) standards, including hand hygiene, PPE use, and isolation precautions throughout each shift.
• Responded promptly to call bells, assisted with repositioning every two hours, and implemented fall-prevention measures to support resident safety.
