OT Case Manager Role for MHSU clients with addiction and complex mental health needs. Developed personalized recovery plans using trauma-informed, strengths-based care to support clients' return to their communities.
- Evaluated functional and cognitive capacity with MoCA UPSA screenings. Completed suicide risk assessments, made safety plans, and arranged psychiatric referrals.
- Facilitated weekly early recovery and art therapy groups using Smart Recovery Principles. Coached clients in daily living, budgeting, planning, and job skills. Collaborated with outreach teams such as ACT, ICM, AOT, ICRT, Harm Reduction, and EPI practice community.

- Assessed daily living activities (ADLs), mobility, seating, and positioning for acute care patients. Used pressure mapping to prevent skin breakdown and aid recovery after surgery. Assisted the SLP during VFSS swallow studies.
- Created and implemented discharge plans, recommending home modifications and adaptive equipment to make patient transitions safer.
- Managed complex hospital repatriation and postoperative protocols for tissue flap surgery and wound care, focusing on strict flap protection, positioning restrictions, gradual progressive loading, seating and positioning and functional independence training. Worked closely with community OT, home, and support teams to maintain continuity of care, ensuring assistance with bathing, dressing, and other ADLs in the community.
Fieldwork (Level 2b): Outpatient Neurorehabilitation at GF Strong, Vancouver Coastal Health (2025-11 – 2025-12)

- Completed upper-extremity and neurocognitive assessments for clients with spinal cord injuries, stroke, ABI and neurodegenerative conditions to guide intervention.
- Implemented advanced interventions such as functional electrical stimulation (FES) for feeding, transcutaneous electrical nerve stimulation (TENS) for pain management, and custom splinting, along with home exercise programs.
- Prescribed specialized mobility equipment and managed complex seating and power wheelchair modifications and driving competency instruction.
Fieldwork (Level 2a): Complex Long-Term Care at Gateway Lodge Assisted Living and Complex Long-Term Care, Northern Health (2024-05 – 2024-06)
- Participated in care planning with various professionals and worked with dietitians and SLPs to support nutrition and feeding. Presented client cases during multidisciplinary care planning, care conferences and family meetings.
- Monitored risks of falls, skin breakdown, and cognitive decline using Scott Fall Risk, Braden Scale, and MoCA. Used results to develop personalized care plans and recommend equipment.
- Assessed lift, transfer, seating, and positioning needs using mat assessments and pressure mapping to keep prioritizing safety, comfort, and skin Integrity.
