Geriatric Emergency Management (GEM) Nurse
Conduct comprehensive geriatric assessments in the emergency department to evaluate cognitive status, functional ability, fall risk, medication safety, mental health, behavioural issues and social supports for older adults. Collaborate with interdisciplinary teams including physicians, social workers, physiotherapists, and community agencies to develop safe discharge plans and ensure continuity of care. Identify high-risk seniors and implement early interventions to prevent hospital admissions, reduce emergency department revisits, and improve patient outcomes.
