- Assist to review of Veterans/Community applicants via Health partners Gateway
- Assist to preparation of new admissions chart
- Prepare new admission required documents prior to admission – obtained from Home Community documents and/or EPIC.
- Assist to actual day of admission process as per Perley Health policy
- Assist to discharge/death process as per Perley Health policy
- Other related home delegated duties
- Perform related nursing duties in LTC setting
- Obtain Physicians’ order and process to EMAR
- Administer prescribe medications orally or by subcutaneous or intramuscular injection
- Initiate admission process and develop initial care plan
- Develop therapeutic relationship with residents and family while providing culturally sensitive care
- Complete referral forms for other disciplines
- Participates with the interdisciplinary team to provide the best possible care for residents and their support systems as they deal with mental health disorders.
