• Assisted residents with activities of daily living that includes, personal & hygiene care, grooming, dressing and feeding.
• Assisted other staff with repositioning, transfers as well as the residents with mobility, ambulation while maintaining patients' safety and dignity.
• Developed therapeutic relationships with the patients using respectful and cultural sensitive communication.
• Prevented falls risk to my best capacity while providing compassionate and patient-centered care within my scope of practice as well as under supervision of the healthcare staff.

Practical Nursing Student - 2nd Clinical Placement at PRHC HOSPITAL - A2, Stroke Unit (2026-05 – 2026-08)

• Maintained patient's privacy and confidentiality in accordance with professional and clinical policies.
• Completed head-to-toe assessments and routine vital signs, recognizing abnormality and reporting it appropriately.
• Participated in med-pass under the supervision of my clinical instructor and assigned nurse.

Practical Nursing Student - 3rd Clinical Placement (In-progress) at PRHC HOSPITAL - B3, Med-surge Unit (2026-09 – Present)

• Provided bed-baths, showers and necessary morning care routine and hygiene while preserving their dignity.
• Communicated to my assigned nurse when changes in the physical condition, skin integrity and overall well-being were observed.
• Applied infection prevention and control principles such as PPE use, hand hygiene and patient-care practices.
• Promote patient safety through falls risk awareness, safe mobility and patient transfers according to hospital policy and professional standard of practice.

Personal Support Worker (PSW) at Bayshore Home Health (2026-01 – 2026-08)

• Assisting clients in maintaining proper body positions to prevent discomfort or pressure sores, especially for those who have limited mobility.
• Changing bed linens and maintaining a clean and comfortable sleeping environment for the client.
• Assisting clients with taking showers, ensuring their safety, and helping them with washing and drying as needed.
• Assisting clients with gentle exercises or range-of-motion activities to help maintain their physical mobility and prevent muscle stiffness.
