We are looking for a candidate with the ability to provide a solution-based approach to housing needs for people who are being discharged from a medical or care settings, linking housing, social care, and medical services to ensure that plans around discharge centre on the transition being made effectively and, in a person, focussed way.
The ability to establish effective working relations operational staff and senior stakeholders across housing NHS and social care, will be key, but also close work with substance misuse, criminal justice, physical and mental health, primary and secondary care and voluntary sector agencies to provide a holistic response.
The postholder will identify barriers which are preventing discharge from facilities, engaging with officers, services and multi-disciplinary teams and work in conjunction with partners to coordinate, develop and deliver positive outcomes for service users.
By reviewing evidence of best practice for therapeutic interventions, working with service users, and bringing co-production into the process, the postholder will also suggest how to improve service pathways for the future.
This is an 18-month pilot post that will allow a case to be made for an ongoing intervention, so the ability to show how services can be provided to get the best outcomes for individuals, and also how this can be done to make best use of resources, will be critical.
