Job summary
An exciting opportunity has arisen for a Practice Pharmacist to join our dynamic and forward-thinking team at Brookside Group Practice. We are looking for 15-20 hours per week.The Practice Pharmacist will be responsible for support to the practice with medication reviews, queries and the delivery of the repeat prescription service. Also, part of the group that implements effective medicine management within the practice, identifying areas for improvement, and initiating and managing change.
Main duties of the job
To provide a medication review and query services to patients via the telephone or face to face clinics as appropriate in the practice.Ensure data is recorded so that patient care is enhanced (eg coding, reminders) and so that it can be retrieved in a meaningful way to help manage patient care and help with data analysis.
It will be necessary to attend and contribute to various practice meetings as requested.

The Practice serves approximately 31,000 patients from our 4 surgeries in Earley, Lower Earley, Winnersh and Wilderness Road.Due to our size, we value communication and teamwork. - Completion of an undergraduate degree in pharmacy and registered with the General Pharmaceutical Council.
- Good standard of education with excellent literacy and numeracy skills.
Desirable
- To hold or be willing to work towards an independent prescribing qualification.
- Have a specialism in diabetes and able to conduct diabetic reviews.
