Responsible for managing and coordinating care for patients on the ward. Duties include clerking and discharging patients, completing advanced nursing assessments, formulation of care plans, completing nurse led ward rounds and assisting and supporting on the weekly doctor ward rounds. Prescribe care and treatment for patients, and advise and support the care team on the ward, whilst working as part of an MDT on a daily basis to ensure patients receive the highest standard of care, with the aim of promoting early, safe and effective discharge in line with the 6 goals of urgent care and care closer to home.
Liaise with consultants in other hospitals to step up the care of patients that cannot be safely managed on the ward. Managing the beds alongside the DLNs to ensure capacity is appropriately achieved and to ensure patients coming onto the ward meet the appropriate criteria to be supported safely.

Nurse Practitioner - ABUHB Urgent Primary Care

Assessment and treatment of patients that present to the department that require prompt treatment and decisions around their care. Patient presentations include those with abdominal pain, UTI, PE, DVT, chest infection, worsening heart failure, gout, back pain, earache, sore throats etc. Onward referral of patients is sometimes necessary for them to receive the appropriate care eg. to surgeons.
Triage is a significant part of the role within urgent primary care and daily involvement in contacting patients that have been referred via the 111 service, to determine if the patient needs to be seen by the service and if not, where is the most appropriate place for them to be seen. This role is very challenging as clinical decisions are made based on assessment over the telephone. Key in taking the lead in developing another arm to the service, to enable nurse practitioners to complete home visits.
This has involved re-developing the standard operating procedure in order for the service to move forward. Service evaluation and poster presentation of findings.

Delivering a hospital at home service working with patients over the age of 18 who are deemed to have frailty and who have complex issues who have become acutely unwell.
