Recently qualified Occupational Therapist and University of Derby graduate with a particular interest in mental health, community practice and human behaviour. My academic studies, clinical placements and previous community experience have developed my interest in evidence-based practice and in understanding the factors that influence behaviour, occupational engagement and meaningful change. I am now seeking to develop this knowledge further through postgraduate study in Behaviour Change.

Coordinated with GPs, district nurses, social workers and palliative care services, advocating for care needs and supporting multidisciplinary care planning. Supported both parents to remain at home in accordance with their wishes, including providing care towards the end of life.

Provided short-term, urgent care and reablement to patients in their own homes to prevent hospital admissions and support safe, timely hospital discharges. Followed appropriate care plans, assisted, supervised and encouraged independent living and sign posted or assisted with arranging appointments, prescriptions, mars sheet and sign posted to district nurse, Occupational therapist, physiotherapists or other professionals if required to do so. Used mobile app for mileage/travel and system one.
All client notes recorded clearly on each visit; observation checks and recordings on News 2 if client felt unwell. Alerted emergency services if required due to illness or falls. Completion of Datix if required to do so.
Professionalism, uniform worn. I worked throughout covid -19 during the crisis so PPE was detrimental. Completed eLearning and required course to be able to perform my role.

Managed inventory flow, monitor stock levels, and coordinate material availability to ensure the production or assembly lines had what they needed on time for picking. Permanent nights shifts. Role came to an end when covid hit.
