Became overall more medically competent, communicating effectively with seniors and other teams and ensuring correct treatment was carried out in a timely manner. Became accustomed to carrying out ascitic drains. Grew in the A&E environment, became more confident in clerking patients, performing procedures and implementing plans.
FY1 Acting SHO - Paediatrics - Paediatrics Department

Required a completely different skill set: assessing and carrying out procedures on children, alongside communicating with parents. Quickly learnt commonly occurring pathologies, paediatric drug calculations and the principles of safeguarding. During HDU shifts, had the opportunity to see and treat rare, chronic inherited disorders with seniors and felt tremendously rewarded when these children made progress.
FY2 - General Surgery - General Surgery Department

Day to day involved surgical ward rounds, assisting in out-patient clinics and in theatres. Learnt about the flow of patient care from pre-op, post-op and then follow-up. 2-3 days of theatre time a week taught theatre etiquette, assisting and completing required paperwork. Registrars allowed performance of incision and drainage procedures regularly independently.
Attended trauma calls for patients in A&E to assess for potential surgical pathology and chase CT traumagrams. Required regular contact with anaesthetics regarding management of rib fractures.
FY2 - Geriatrics/General Medicine with A&E Hot Block - Geriatrics and General Medicine Department

Learnt treatment pathways for end of life care, community care, physiotherapy and nutrition alongside treating medically complex patients. Daily ward rounds involved constructing detailed management plans with seniors, enabling holistic care of patients with multiple co-morbidities and poly-pharmacy. Weekly discussions with family and careful communication skills were required in this slower-paced environment. 4-6 week 'hot block' in A&E involved clerking patients and working with seniors to ensure correct treatment in a timely manner.
FY2 - Obstetrics & Gynaecology - Obstetrics and Gynaecology Department

Weekly participation in vaginal and c-section deliveries, urogynae theatres and women's health clinics. Obstetrics side was extremely senior-led; opportunities were very useful in understanding common presentations and pathologies in pregnancy, labour-related complications and management. Gynaecology side was more hands on for SHOs and extremely busy.
Responsible for the flow of patients between out-patient triage, A&E and the women's Health Clinic on a regular basis. Important to have a low threshold to seek senior advice and understand the protocols in place for most common presentations.
