I am responsible for applying public laws, rules, regulations and exclusions governing confidentiality of the medical record, including the Privacy Act, Freedom of Information Act (FOIA) and 38 U.S.C. 7332 governing the release of records containing information regarding the treatment of or referral for drug and/or alcohol abuse, Sickle Cell Anemia, and infection with Human Immunodeficiency Virus (HIV). I process all incoming requests to the facility for Release of Information (ROI). My duties also entail evaluating the validity of each request, determining which information is to be released in compliance with existing laws (Privacy Act of 1974, FOIA, and Health Insurance Portability and Accountability Act).
I also ensure that proper authorization exists before release is made and inputting all requests into eROI+ for logging and tracking of these requests. I sorted, scanned and indexed medical records for several Bon Secours Mercy Health facilities. I used ConnectCare to locate patients and enter their records into the system; our EMR system changed to EPIC, and I now utilize that system to enter patient information. I was responsible for adjudicating Title 2 and Title 16 adult disability claims on behalf of the Social Security Administration (SSA). I would compose letters to claimants, and compose a medical history analysis/summary, based on the claimant's medical and work history and their ADL's. I supervised and coordinated tasks for an hourly worker that worked in my close chart room daily. Other tasks include archiving paper charts, filing, and working closely with the clinicians and County Attorney.
I was also responsible for ordering chart room supplies and maintaining the supply closets, archiving financial records for the finance department, and maintaining the closed chart room.
Medical Records Assistant at Patient First (2016-02 – 2017-05)
My job responsibilities included answering incoming calls from patients, doctor's offices, nurses, centers and attorney offices, that is calling to either have medical records faxed to them or are checking the status of a pending medical records request. I was responsible for verifying the HIPAA authorization with reps from PSS and letting them know if the person they have on the other line has a valid authorization on file. My job included placing outbound calls to doctor's offices to ensure that I will be faxing the records to the correct fax number on a patient's behalf.
Light administrative work as assigned. I also complete and update the missing information sheet for patient's that call in to our office in reference to have received a PCP Letter in the mail. I also took payments for medical records from vendors.
Senior Medical Records Rep at CIOX Health (2015-08 – 2016-02)
My job duties included processing medical records requests for Overland Park Hospital, Lee's Summit Medical Center and Spotsylvania Hospital. I was responsible for answering legal questions and processing certifications for hospital directors and lawyers. I was also responsible for ensuring that I adhere to the HIPAA laws when processing a medical records request and preparing a correspondence to the requestors if there was missing information or if a second identifier is needed in order to process their request.
I would take incoming calls and place outgoing calls to attorneys, hospital directors and patients. I operate a scanner, fax machine, printer and two computer monitors. I also created spreadsheets in MS Excel to give to the hospital directors, so they will know which requests are pending for their site.
