Completing functional assessments using agreed protocols and as directed by senior therapist including washing and dressing, shower assessments, kitchen assessments, progression of mobility and home exercise programmes, balance programmes and mental health intervention.
Assisting in assessing and planning Social Care / Packages of Care and referrals under the guidance of the occupational therapist.
Completing the initial basic home assessment.
Managing patient data for example: Careflow, Starting discharge letters, Evolve, Patient centre, Vital Pak, Easy (mileage claims),Remote access.
Completing Data collection and transferring information onto a spread sheet using Microsoft Exell
Safe handling of medical notes using Trust system SENA
Undertaking non-clinical duties related to the treatment and management of patients, ordering/delivering equipment, recording patient contacts, arranging referrals to other Using the Millbrook system ordering necessary equipment when required
NVQ 3 in Adult Social Care
