Completing functional assessments using agreed protocols and as directed by senior therapist including washing and dressing, shower assessments, kitchen assessments, progression of mobility and home exercise programmes, balance programmes and mental health intervention.

• Adjusting functional treatment goals with the patient according to progress made or changes accrued.

Assisting in assessing and planning Social Care / Packages of Care and referrals under the guidance of the occupational therapist.

• To carry out rehabilitation as determined by patients’ needs.
• To provide minor adaptations to the patients home for example measuring for rails.
• To undertake routine observations and act appropriately on the results of blood pressure, temperature, pulse and respiratory rate, oxygen saturation levels. Refer to nurse in charge as appropriate.
• Monitor and record appropriately bowel actions, act on concerns for example constipation or diarrhoea, report to the sister any concerns.
• Obtain urine sample if assessment indicated and analyse sample as per hospital protocol.
• Advice on the use and provision of activities of daily living and other equipment.

Managing patient data for example: Careflow, Starting discharge letters, Evolve, Patient centre, Vital Pak, Easy (mileage claims),Remote access.
