Worked as a Community Nurse within the Family Health Team supporting health visitors and working closely with other professionals such as school nurses, doctors, nurseries and schools to ensure continued care for the child. A key part of this role involved safeguarding responsibilities, which was recognising concerns and following safeguarding protocols and making referrals to MARU if necessary. Developed strong mental health awareness which added to knowledge, and supported parents experiencing emotional or mental health difficulties following childbirth. Gained knowledge of Adverse Childhood Experiences (ACES) and their long term impact on someone's wellbeing.
Responsibilities included completing assessments for parents and babies, running weekly clinics for child development and looking for signs of abuse, neglect or vulnerability. Regularly worked with vulnerable families and had to keep records confidential and in line with the data protection act. Had to follow the Healthy Child Programme and also lone working policies checking if any risk assessments were in place to maintain safety at all times. This job strengthened communication, safeguarding and emotional support skills.

Worked with people with learning disabilities aged 16-25 years old in the Transitions team. A lot of referrals were made to different specialists, including epilepsy, PEG and dysphasia. Making confidential notes was essential, and keeping these notes safe while in transport.
Worked alongside health care assistant and senior nurse. Health promotion was part of the job, which in team was promoting purple folder (a document which someone with Learning disabilities could take with them to any professional and it provided them with essential information). Had a caseload of clients and had to complete assessments, liaise between children's and adult services and GPs.
The team worked very closely with the social workers and often work was completed together. The role of the community learning disability nurse was to ensure the health needs of the service users are being met. All work had to be within the Herts County Council guidelines and also by following the NMC code.

Worked in a residential home for people with learning disabilities. Duties included implementing care plans for the service users and risk assessments. Cooking and cleaning were performed on a daily basis and interacting with the service users, taking out in the community or playing games in the lounge. Administering medications on a daily basis and documenting these. Also documenting any marks to service users that may be found on their body - by body mapping it and sending the forms off to be assessed. Part of role was to work with health care assistants and to do hand overs at each shift making sure all information was passed on to the next staff members.
