As a General Practice Nurse, I provide high-quality patient care in a primary care setting, including administering immunisations and vaccinations, managing long-term conditions, and delivering evidence-based advice to support patients' wellbeing. During this placement, I supported nurses and healthcare assistants with personal care; I administered medication, I administered oxygen, BIPAP, CPAP, HIFLO and more. I cared for patients with COVID, tuberculosis, COPD, asthma and many more respiratory conditions. I supported the nurses with documentation and regularly managed a bay with the support of the nurses and the healthcare assistants.

During my time at rapid response I worked closely with the nurses and other healthcare professionals to prevent hospital admissions if possible and to assess patients using auscultation, top to toe assessments, observations and more. We would discuss the patients symptoms and the possible diagnosis before calling the patients GP and discussing our concerns. We would discuss medication necessary for such illness or infection etc.
In the case that a patient was deteriorating, we would call the ambulance and wait with the patient until they arrive, supporting the patient in the best way that we could.

My placement at St Raphaels Hospice was focused on building close relationships with patients and making them as comfortable as possible. I spent time talking to my patients and getting to know them, I administered medications including via syringe driver, I provided personal care, I made referrals and I communicated with the patients and their families about their care and their options etc. I worked closely with nurses, nurse associates, health care assistants, doctors, physician associates and more to provide the best care for each patient.
I worked alongside the palliative care nurses, providing patients pain management and EOL care.
