Social Worker – Contact Cares, St Helens Borough Council

• Conduct adult social care assessments to support discharge within an Intermediate Rehabilitation Facility.
• Serve as the adult social care representative within a multidisciplinary team, supporting clients through rehabilitation.
• Liaise with families, professionals, care arrangers, and housing services to streamline discharge processes.
• Implement evidence-based, person-centered assessments under legislation such as the Care Act, Mental Health Act, and Mental Capacity Act.
