• Support people leave the hospital and get back to living independently in their own homes
• Support to regain and gain skills in daily living ie personal care, dressing, meal preparation etc.
• Recommend the use of equipment to improve mobility and the introduction of TECS (Technology Enabled Care Services) to support independence with daily living tasks i.e medication and appointment reminders.
• Supporting with household tasks such as shopping, cleaning and cooking, in a person-centred approach to empower the individual and promote independence.
