Providing safe, compassionate and person-centred nursing care to patients in their own homes and care settings.

• Carrying out holistic assessment of patients' psychological, physical and social needs
• Safe administration of medication as prescribed and in accordance with policies and professional standards
• Carrying out wound assessment and providing appropriate wound care and dressing changes
• Prevention and management of pressure ulcers, through regular skin assessments, repositioning advice and appropriate interventions
• Provide catheter care, continence and personal care, maintaining the patient's dignity, independence and privacy
• Monitoring vital signs, recognising changes, and escalating concerns appropriately to senior nurses, G.P., and other health care professionals
• Supporting patients with complex and long-term conditions in self-management and wellbeing
• Providing compassionate care and support to patients approaching the end of their lives and their families
• Assisting patients with activities of daily living, mobility, nutrition and hydration where required
