As part of my role working across nursing and residential homes, I provide advanced clinical care to improve clinical practice, resident outcomes, and overall quality of care. In addition to clinical nursing tasks / skills as part of my role I work an autonomous practitioner to effectively assess, diagnoses and complete clinical management plans for acutely unwell patients within their home / care home/ nursing home setting with the aim to avoid admission and act on findings as per agreed protocols. As part of the role, I will obtain a complete history, medication review and a complete a full head to toe physical assessment on my patients. I will analyse blood results to support diagnoses and liaise with the wider MDT to create clinical management plan.

I have good working knowledge of all the highly specialised clinical services provided within health, considerable concentration, and ability to analyse each situation and make appropriate judgements for the more complex patients and have good time management skills.
