In my current role as a Specialist Community Public Health Nurse within Hampshire & Isle of Wight NHS Trust, I undertake comprehensive holistic assessments across universal, targeted and enhanced pathways. I assess risk, vulnerability and safeguarding concerns, synthesising complex information to inform robust care planning. I regularly lead and contribute to multidisciplinary meetings including Child in Need, Child Protection, Team Around the Child and MARAC panels. These experiences have strengthened my analytical skills, ability to present defensible assessments, and confidence contributing to structured decision-making processes — all transferable to Decision Support Tool completion and CHC assessment requirements.
Alongside my community role, I practise as a Band 5 Registered Nurse within an acute hospital environment. I manage patients with complex long-term conditions, multi-morbidity and acute deterioration. I am confident in systematic clinical assessment, recognising deterioration, escalating concerns appropriately and contributing to safe discharge planning. This has provided insight into Discharge to Assess pathways and the importance of coordinated working between acute, community and social care services to ensure safe transitions of care.
I am fully conversant with the assess, plan, implement and evaluate cycle and ensure all documentation is clear, accurate and aligned with governance standards and the NMC Code. I maintain professional curiosity and am confident providing professional challenge where quality or safety concerns are identified. My safeguarding experience has strengthened my resilience and ability to manage sensitive or complex situations with objectivity and compassion.
My previous background in production planning developed strong organisational, analytical and coordination skills. I am comfortable facilitating meetings, managing competing priorities and ensuring processes are completed within required timeframes. These transferable skills support effective MDT facilitation, case management and adherence to CHC timescales.
I am resilient, adaptable and able to work autonomously while maintaining effective collaboration with multidisciplinary colleagues and Local Authority partners. I hold full NMC registration as both an Adult Nurse and SCPHN Health Visitor, maintain up-to-date professional development and hold Safeguarding Level 3 training. I have access to a vehicle and am able to travel across the ICB region as required.
