Carry out drug histories on new patients admitted into the hospital, mainly specialising on the kidney and liver transplant wards as well as the intensive care wards. Work on transplant wards has provided insight into the importance of being vigilant and careful in handling and dealing with patients as many are going through emotional distress. Not only carry out a drug history but also build a trusting relationship with patients.
Assisted in creating a training package for other staff members to understand and learn about working life on the liver, kidney transplant and intensive care units and the steps needed to carry out a thorough drug history. Assist in the discharge process ensuring patients are sent home with the correct amount of medicines and are counselled on any new medicines they are now taking. Accuracy check medicines in the dispensary and labelling rooms for the wards.
Take part in the training process of new staff members. On a 4 weekly rotational basis take on the coordinating role.
