Patient Facing Clinical Services Care. Full time working remotely and independently. Managing a caseload of complex patients as required by GP Practice.
- Prescribe ADHD repeat medications as per shared care plan
- Arrange baseline physical checks and monitor for changes in heart rate, blood pressure, and mental state
- Conducting remote medication reviews and agree to medicines care plans in agreement with patients and carers
- Liaise with local and distance pharmacies to discuss stock availability and any queries raised by other healthcare professionals
- Liaise with MDT, providing clinical input and agree to shared cared plan
- Delivering long-term conditions clinics and prescribing accordingly within competency and agreed management plans under the supervision of GP
- Consult with patients for acute and long-term illnesses within defined levels of competence and prescribe medication within agreed management plans under the supervision of a GP
- Prescribe appropriate medication for patient needs within areas of competency and within agreed management plans

- Manage therapeutic drug monitoring system and recall of patients taking high risk drugs i.e. anticoagulants, anticonvulsants, DMARDs and diabetic medications etc
- Organise and oversee repeat prescribing, repeat dispensing, electronic prescribing and medication review systems with the help of mentor GP
- Handle prescription queries and requests directly
- Oversee system for reconciliation of medicines from outpatient and discharge letters and support safe transfer of care through liaison with hospital and community colleagues
- Liaise with other stakeholders in the supply chain, such as community pharmacists and appliance contractors to handle queries and manage supply problems

Responsible for the planning, organisation, implementation, and ongoing development of the EPMA system.
- Tailoring psychiatric medication regimens (e.g., antidepressants, antipsychotics) to maximize effectiveness, adjusting dosages, and minimizing side effects
- Conducting therapeutic drug monitoring for complex or high-risk treatments like lithium and clozapine
- Working closely with the EPMA Team and the EPMA software provider in the creation, updating, and maintenance of appropriate system documentation
- Lead on the implementation and delivery of specific EPMA within the Medicines and Pharmacy Transformation agenda
- Ensuring the EPMA business continuity plan is developed to give the highest level of assurance that patients can continue to be treated safely in the event of EPMA system failures or catastrophic failure
- Developing a high level of understanding of the clinical requirements and technical capabilities of the EPMA, thus ensuring that maximum clinical, risk reduction, efficiency, and financial benefits are derived from the system

- Working closely with the Trust IT Trainers and the EPMA providers in the development, content, review, delivery, and updating of all EPMA test plans
- Ensuring that EPMA is aligned to support the delivery of Medicines Optimisation objectives within both the Trusts' Clinical Strategy and overall Quality Strategy
- To continually optimise the EPMA system in terms of configuration, development of prescribing and administration tools, and in response to local incidents, national best practice or relevant research results within the EPMA field to support clinical staff to deliver safe and effective patient care
- Leading the EPMA Team, IT Trainers and the EPMA software provider in the proactive design, development, testing, implementation, and quality assurance of new functionalities within the EPMA
- Ensuring that the EPMA system supports all aspects of Controlled Drug usage and delivery of relevant information requirements to the Controlled Drugs Accountable Officer
- Ensuring that the EPMA contains medicines information that is current, accurate, and clinically appropriate, including maintaining the medicines file (including appropriate nomenclature, formulary status, formulations, routes of administration, doses, frequencies, administration details, etc)
- Working closely with the Trust IT department to ensure that the EPMA is secure from unauthorised or inappropriate access (including authorising new user access, maintaining user accounts and appropriate access rights, removing users if they leave an area or access rights change), and monitoring/auditing use of the EPMA
- Evaluating the clinical appropriateness, safety, and legality of prescriptions based upon the patients' diseases, co-morbidities, and other medication and treatments taken, contacting the prescriber to make agreed changes as appropriate
- Participating in the Drug & therapeutic committee meetings for monitoring and reviewing of the policies, and for the approval of any changes

- Develop a close working relationship with members of the multi-disciplinary team and contribute to the clinical care plan
