I am now seeking a role in domiciliary care where I can focus on providing hands-on, compassionate support to people in their own homes. I have extensive experience supporting individuals with all activities of daily living, including washing, dressing, personal care, mobility, and medication administration.
I am passionate about helping people maintain their dignity, independence, and comfort within their own environment. Band 5 Staff Nurse - Northern Care Alliance (NCA) - Bury
Current role working part time 24 hrs (12.5 hrs over 2 days) in intermediate care unit in Bury. Working with Bury council and NCA using health and social care workings in 36 bedded unit. Working with GPs OOHs ANPs using clinical assessment and hospital avoidance ethos being able to plan individualised support care plans working with clients to reorientate to own home for discharge.
Working with pharmacy team in administering safe medications with patients and assessing for self-medication. Working with collaborative team working with social workers reablement teams and therapy and care staff ensuring high standard of care and focus for rehab/or discharge to long term care setting. Completing RCAs and presenting to leadership team for improvement in care.
Active in Dementia task group looking at implementing care standards and methods of assessment introducing 4T and implementing person centred care.
