I conducted nutritional assessments for adults with learning disabilities using measurements and dietary histories to gain a comprehensive understanding. I created personalized nutrition care plans addressing weight management, malnutrition, diabetes, constipation, and sensory-related eating difficulties. I shared nutrition advice in an easy-to-understand format and managed a community caseload, prioritising referrals and timely follow-ups.

Collaborating with multidisciplinary teams, I advocated for clients' nutritional needs while ensuring that safeguarding and nutritional concerns were reported appropriately. I maintained accurate clinical records in line with Trust policies and participated in audits and service development, dedicating time to clinical supervision and professional development.

Band 6 Mental Health Dietitian – 3-month AHP pilot Project - East London Foundation Trust (ELFT) - East London

I provided dietetic support to adults facing mental health challenges within ELFT inpatient services. I effectively managed a complex caseload, applying clinical reasoning for assessments and interventions. Additionally, I contributed to delivering Nutrition Screening and Care Planning Training for staff, facilitated referrals and discharge planning, and participated in health promotion and quality improvement initiatives within the ward.
