- Identification of issues of quality in health systems within the cluster (18 facilities and hospitals) including clinical services through establish a baseline needs assessment
- Mentor and couch Clinical Staff on Quality Improve through joint facility visit with DoH personnel (PHC managers, Clinical Programme Coordinators)
- Conduct weekly feedback meetings with Clinical advisor to review QIPs and progress on facility performance
- Mentor and couch facilities in implementation of Operation Phuthuma approach and QIP development
- Support the DoH and HST teams in identifying performance gaps through data analysis, and develop QIPs at facility and sub-district level
- Conduct site visits with DoH and stakeholder counterparts to perform assessments progress on implementation plans and quality improvement plans (QIPs)
- Provide technical support in the Sub-District cluster in tracking performance of all HAST/TB indicators
- Assist in data quality assessment in facilities using data management sources (Tier, Net and files to triangulate data and improve quality of data)
- Conduct Clinical file audits in Pediatrics and adults to monitor guidelines implementation, identify compliance in documentation and develop quality improvement
- Identify skills gaps in the allocated sub-district(s), and create and implement a skill development plan for individuals in the DoH and HST teams
- Conduct in-service training on relevant identified gaps on guidelines
- Coach and mentor DoH and internal staff programmatically
- Support PHC (Managers,) in establishing relationship, collaboration and mapping of all stakeholder within the Sub District
- Support and participate monthly and Quarterly stakeholders meeting in a Sub district Level
- Collaborate with relevant stakeholders within the sub district in supporting the DoH facility in achieving HAST/TB indicators (95. 95.95 strategy)
- Conduct facility visit and engage with EXCO (CEOs, Operational, PHC Managers, M&E managers, Clinical Managers, FIOs and Systems Manager.)
- Conduct stakeholders meeting on weekly and monthly basis to discuss performance based on indicators
- Build and maintain relationships with other stakeholders (the DoH; Departments of Social Development, Education, Arts and Culture, and Agriculture; District and Local AIDS Councils, Operation Sukuma Sakhe structures, District and Sub-District Health Management teams within the Supported District
- Collaborate and engage with HST internal stakeholder in supporting the DoH facility (UB Team Synch team, CCMDD Team and Cross Site Coordinators)
- Support PHC Managers in strengthening of Community Health Workers and Ward based Outreach Team and referral systems for improved of linkage to care
- Overall staff supervision in Sub District (Clinical Nursing staff (Enrolled, Professional Nurses) data clerks, lay counselors, drivers and Psychosocial Advisors) in the 18 supported facilities
- Management staff conflict and disciplinary issues in line with policies and procedures
- Conduct staff probation/performance reviews every six months in sub-district
- Develop Sub District work plan in line with DOH
- Develop and implement sub-district project work plan, and aligned with the project work plan
- Monitor and evaluates weekly monthly, Quarterly and annually Sub-District Performance through OP facility and Sub District meetings
- Review and conduct facility data analysis with M&E officer to monitor the progress and identify facilities that are under performing
