Field Coordinator - Patient Provider Support Agency (PPSA)

• Conduct the situational assessment activities during project initiation and update them on a periodic basis and making Data Reporting Monthly shared to DTO and State Team
• Mapping of public and private health facilities, and informal practitioners in the district that provide services, and engage them through technical support activities and Making Facilities ID
• Facilitate the establishment of Hub sites in Public & Private identified facilities and ensure the proper functioning of the same and Solving Issues
• Documentioned of Monthly Notified Patient for Nutrition Support for Bank and other Details. Submit to State Team. And District Level
• Build rapport and advocacy at the PLL generation of monthly making and checked all PHA indicators against all Patient on Nikshay Application. And matching to all indicators for PLL claimed checked to compulsory
• Coordinate and ensure for the sample transportation for all the identified persons with Presumptive and UDST with PHA for Neutrino TB in the Hub with Spoke Sites
• Conduct capacity building and sensitization of frontline healthcare providers and all staff Trained to Nikshay
• Coordinate with NTEP for contact tracing, arranging camps, and participating in ACF Activities and tracking to Daily Reporting by Team
• Engage the TB Champions for raising community awareness and mobilization, Addressing stigma and follow-up of the adherence tracking and documenting their Contributions
• Follow-up a robust tracking mechanism of the diagnosed TB cases in the hub sites for their Diagnosis, treatment adherence and ensure its completion successfully
• Maintain records, collect data, and compile program activity information, while also Attending NTEP FDC and health Data at both District Levels
• Document the progress of performance indicators outlined in the project and represent them in various community and District DHS. Meeting
