As a hospital pharmacist, i were responsible for ensuring patient safety and optimal therapeutic outcomes by accurately verifying physician medication orders, performing clinical interventions, and managing complex drug therapies like sterile IV compounding and chemotherapy. I collaborated closely with multidisciplinary healthcare teams during patient rounds to provide expert drug information and dosing recommendations, while also conducting comprehensive medication reconciliations at admission and discharge to prevent errors. Additionally, i supervised pharmacy technicians, managed automated dispensing systems and controlled substance inventories, and provided vital bedside counseling to patients to improve medication adherence and overall recovery
