• Assembling, retrieving and filing of Medical records
• To check and supervise that all medical records receive, checked and stored systematically for easy irretrievability
• Ensure the medical records are complete, legible compiled and arranged in a predefined sequential manner
• Maintains record system for follow-up patient care
• Filing of Insurance forms
• Secures information for special projects, such as Cancer Registry or reports for accreditation agencies
• Secures information from patients' medical records for release to authorized individuals or agencies Attend court cases
• ICD-10coding of diseases dealing with patient, attendants, T. P.A quarries etc.Coordinating with various internal departments
• Prepare and compile data & reports as & when required
• Maintain in IP & OPD Record
• Maintain in Quality Indicator, ALOS, and BOR data for BPL patients
• Coding & Indexing as WHO International Statistical Classification Of Diseases and Related Health Problem (ICD -10)
• Preparing Daily Census
• Maintain in Death & Birth & MLC Registers
• Completion of Insurance Claim Form, Certificate & any types of enquiry related to MRD
• Preparing Slides for Presentation
• Scanning files of IPD
• Deficiency, Checklist, Completion of IPD Files
• Online intimation of Birth & Death to MCD on daily basis or issued reference slip to attendant for collection of certificate form Zonal MCD & MCG
• Prepare communicable, Non-communicable, IDSP and others Govt Reports
• All procedure related to medical records and health information management
