Key Achievements:
- Complete five home visits each month for 15 families throughout my area of Riruta and Satellite slums.
- Complete ten school visits for monitoring the progress of the students in school within one term.
- Manage and developed volunteer programs including intergenerational groups.
- Developed and implemented individualized care plans.
- Intervened in crises using crisis management and de-escalation techniques for the youth.
- Designed, implemented, and monitored behavior modification plans for the students.
Responsibilities:
- Conduct home visits to do needs assessment for children’s beneficiaries to put in the sponsorship program
- Responsible for choosing the right intervention to make sure the organizational goals for access and retention in primary and secondary school and university for needy children are met.
- Conduct school visits to see how the students are faring on with their studies to update the sponsor.
- Analyze the identified cases and makes an implementation plan.
- Make referrals depending on the Childs need and complexity of the case.
- Approaches schools and parents to advocate for children’s rights and needs.
- Compile case reports.
- Ensure students have written their correspondences to the sponsor and upload them on the internet for the sponsor to read and reply to the child.
- Ensure parents and students sign consent forms due to the children’s pictures being used in the organizations website.
- Monitor the post-secondary students and the Alumni students in their work places.
- Provide psychosocial support to the children and youth affected by various issues within the slum and family and make referrals.
