A dedicated and professional clinical pharmacist with 1.5 years of experience in clinical pharmacy and 3+ years of experience in the community pharmacy. Discuss patients past medical and medication histories, and validate the medications prescribed with its posology before and during admission according to the guidelines’ recommendations
Report interventions and describe problems to physicians, residents, and nurses related to the side effects,
dose adjustments, and drug-drug interactions while monitoring each semester’s floors’ medication reserves’
