Worked with the Clinical and M&E Backstops of assigned Facilities to manage client data, EMR operations, and health facility coordination.
- Interaction with clients at the health facility to collect their baseline data (enrollment) of clients in the Facility and community on source documents and on the EMR.
- Engage with the client via physical communication on the need for Biometric capture for TX New and recapture for clients with NO Match outcome.
- EMR data entries to update refills, viral load sample collection and results.
- Generate reports from the EMR such as RADET files, AHD, Biometric report, HTS report, PMTCT Maternal Cohort, patient line list etc.
- Triangulation of Enrolment Registers, ART Registers, Presumptive TB screening Register with Data on the EMR.
- Ensure linkage of all clients to either the facility hub or any other close by facility for continuity in care.
- Ensure 100% tracking of clients who missed their appointments.
- Update client status during revisit for CD4 test, pharmacy drugs refill.
- Provide support to field officers in HCT Services at the community.
- Ensure routine EMR Use, check for and apply updates, when necessary, run LAMISPLUS DQA routinely and clear all Data quality flags in the LAMISPLUS database.
- Ensure 100% Daily/weekly/Monthly update, completion and validity of HFR Data, comparing Daily entries with Monthly Data in the Service Registers/MSF.
- Ensured clients biometric is captured and recaptured.
