• Perform final clinical, legal and accuracy checks across two high-intensity pharmacies, reviewing approximately 400 dispensed items per day across prescription, Hi-Tech, controlled-drug, cold-chain and walk-in workflows.
• Apply structured prescription triage covering legal validity, patient identity, drug/class, dose, strength, form, quantity, indication, allergies, interactions, PMR history, scheme compliance and counselling requirements.
• Identify and resolve high-risk medication issues including duplicate anticoagulant therapy, incorrect methotrexate directions, and increased bleeding risk from aspirin/SSRI/NSAID combinations, documenting interventions and escalating to prescribers where required.
• Manage regulated medicine workflows including controlled drugs, methadone/OST, phased dispensing, emergency supplies, Hi-Tech medicines, owings, returns and cold-chain products such as vaccines, insulins, GLP-1 agonists, biologics and immunosuppressants.
• Implemented SOP aids, checking prompts and team training tools for high-risk dispensing, cold-chain handling, PCRS/Hi-Tech processing and near-miss documentation, improving workflow consistency and reducing repeat error patterns.
