Behavioral Health Nurse Practitioner

People's Health Clinic | Park City, Utah

Full-time, 40 hours per week with 30 hours of patient-facing time per week | Reports to the Chief Medical Officer, with clinical supervision from the Behavioral Health Medical Director

Position Summary

PHC seeks a Nurse Practitioner to join our behavioral health team in a clinically focused role. You will provide behavioral health assessment, counseling, and medication management for our patients, working in collaboration with and under the clinical supervision of our Behavioral Health Medical Director to deliver care tailored to the unique needs of an uninsured, largely Spanish-speaking population.

You will work alongside PHC's adult primary care, pediatrics, and women's health teams to support an integrated care model in which behavioral health is woven into every patient's care rather than siloed from it. This role is well suited to a Nurse Practitioner who is building expertise in behavioral health and pursuing Psychiatric-Mental Health Nurse Practitioner (PMHNP) certification, with clinical supervision and support along the way.

Key Responsibilities

Clinical Care

  • Provide outpatient behavioral health assessment, counseling, and treatment, including medication management, in collaboration with and under the clinical supervision of the Behavioral Health Medical Director
  • Recognize behavioral health emergencies and initiate appropriate triage and referral
  • Maintain accurate, timely clinical documentation consistent with PHC standards
Integrated and Collaborative Care
  • Work closely with PHC's adult primary care, pediatrics, and women's health teams to support integrated care, including shared care planning and warm handoffs
  • Participate in collaborative, interdisciplinary case reviews with PHC providers
  • Coordinate with community agencies and mental health partners across Summit and Wasatch Counties to support referrals and wraparound care
  • Contribute to quality improvement efforts and the collection of behavioral health outcome data
Patient Advocacy
  • Advocate for patients with attentiveness to social determinants of health, health inequities, and the challenges faced by underserved populations
What Success Looks Like Within the first year, the Behavioral Health Nurse Practitioner will have:
  • Established a consistent panel of behavioral health patients receiving timely, high-quality care
  • Strengthened warm handoffs and shared care planning with PHC's primary care, pediatrics, and women's health teams
  • Contributed to a patient experience in which behavioral health feels integrated, not siloed
  • Grown in clinical confidence and scope of behavioral health practice
Qualifications

Required

  • Graduation from an accredited College of Nursing
  • Current licensure as an Advanced Practice Registered Nurse / Nurse Practitioner in the State of Utah, with national certification as required under state law
  • Prescriptive authority and active DEA registration, or eligibility
  • Commitment to developing clinical expertise in behavioral health
Preferred
  • Psychiatric-Mental Health training, or active pursuit of PMHNP certification
  • Spanish language proficiency
  • Experience serving uninsured, immigrant, or otherwise underserved populations
  • Experience in outpatient or integrated behavioral health settings
