Job description
DUTIES AND RESPONSIBILITIES
- Ensure all medical record information is protected and kept confidential.
- Fill medical records upon request for all departments and locations of the clinic to persons and agencies according to state and federal regulations.
- Responsible for assuring that medical records are received, reviewed, processed, scanned, and filed on a timely basis.
- Review medical records for completeness, accuracy, and compliance with regulations.
- Assure all information released to persons or agencies are documented in the Electronic Health Record (EHR).
- Assist patients and staff fill out medical record forms and send them to previous providers.
- Process/track invoices (A/P & A/R) for medical record fill charges.
- Answers telephone and takes request for medical records or information while maintaining patient/client confidentiality.
- Demonstrate knowledge of legal aspects of medical record administration.
- Promote effective working relationships and work effectively as part of a team to facilitate the Clinic’s ability to meet its goals and objectives.
- Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous work environment.
- Respond to ROI (release of information) requests for medical records.
- Validate requests and authorizations for the release of medical information.
- Ensure that clinical documentation of the services provided to our patients is correct.
- Other duties as assigned.

EDUCATION AND EXPERIENCE
- High School Diploma or GED required, or minimum two years’ work in a related field.
- One-two years of experience in administration, processing and managing medical records required.
- Bilingual Spanish and English fluent, preferred
TYPICAL PHYSICAL DEMANDS
- Prolonged sitting or standing may be required.
- Ability to lift 25 pounds; occasionally up to 60 pounds.
- Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with normal medical office will be performed on a repetitive basis.
- The ability to distinguish letters and symbols as well as the ability to utilize telephones, computer terminals and copiers is required.
- Work under stressful conditions as well as irregular hours may be required.
- Frequent exposure to communicable disease, body fluids, toxic substances, medicinal preparations, radiation, and other conditions common to a clinical environment may routinely be encountered.
