Job Summary: Intention Healthcare is seeking an experienced and mission driven Medical Director to oversee and guide the Intention Provider team in delivering home-based primary care across Massachusetts. This leadership role ensures the delivery of high-quality clinical services through direct oversight of provider performance, documentation standards, quality initiatives, regulatory compliance, and clinical best practices.
· Serve as the primary point of contact for clinical questions, diagnostic guidance, and case consultations.
· Ensure adherence to evidence based clinical standards, treatment protocols and organizational policies
· Conduct periodic ride-alongs, case audits and chart reviews to monitor quality and consistency of clinical care
· Lead quality improvement initiatives aimed at clinical effectiveness, patient satisfaction and decreasing utilization rates
· Develop and implement workflows, best practices and quality benchmarks to optimize operational efficiency
· Provide feedback, coaching and performance evaluations for providers including individualized improvement plans when needed.
· Ensure closed loop clinical communication occurs with external clients, partners and care team members
· Participate in strategic planning for clinical service expansion, program design and innovation in home based care
· Support onboarding, training and continuing education for Intention Providers
· Diagnose medical conditions, develop individualized care plans, and prescribe medications as appropriate.
· Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care.
· Maintain detailed and accurate electronic health records (EHR) of patient assessments, treatments, and follow-ups.
· Successful completion of residency in internal medicine or family medicine at an accredited healthcare institution
· Current Massachusetts license and prescriptive authority
· Board Certification in Internal Medicine or Family Medicine
· Geriatrics training or palliative care experience is preferred but not required
· Minimum of 5-7 years of clinical experience, preferably in home-based care, primary care, geriatrics, internal medicine or complex disease management
· Demonstrated leadership experience
· Proficiency with EMR/EHR systems and documentation standards
Potential to purchase shares in company available for tenured providers
Pay: $275,000.00 - $350,000.00 per year
