Physical Therapist - Per Diem

BrightStar Care of Scott and Carver Counties is seeking a Per Diem Physical Therapist (PT) to work with our Home Health Care clients and provide in-home therapy services. Our clients are residents of Scott and Carver Counties, many of which are Veterans. As a PT with BrightStar Care Scott and Carver Counties you must be motivated, serve with passion, and a desire for a flexible schedule. This position will work in the areas including but not limited to Elko, Lakeville, Webster, Rosemount, Farmington, Jordan, Belle Plaine and New Prague.

Position Responsibilities:

· Provide Physical therapy services to clients according to physicians’ written orders.

· Assess and evaluate therapeutic/rehabilitative/functional status.

· Develop a plan of care and treatment making sure the plan of care is consistent with the assessment.

· Assess the home environment specifically as it relates to fall risk and interventions and home safety.

· Report client’s response to treatment or changes in condition to Director of Nursing and physician.

· Instruct other personnel and/or family members on goals.

· Assist in identifying client and family/caregiver needs for other services.

· Prepare and submit clinical progress summaries based on identified and attained goals.

· Assure compliance with all local, state, and federal regulations.

· Collaborate with the interdisciplinary team.

Required Qualifications:

· Graduated from a Physical Therapy Program and currently licensed in the State of Minnesota.

· Two years of appropriate experience as a Physical Therapist, home health care is preferred.

· The ability to communicate with physicians and medical staff regarding patients care and current status.

· The ability to travel within our territory (Scott and Carver Counties, MN).

· Must have reliable transportation.

Pay: $46.45 - $55.94 per hour
