I perform assessments on potential admissions for our unit by going with a counselor to our ER department. I also assess, obtain vitals and medicate our clients while on the unit, report back to the Dr any abnormal findings. I take time to talk to and get to know them when timing allows.
Sometimes all they want is someone to hear them. When it's time for them to successfully discharge from our facility I call in prescribed medications to their pharmacy or send over any information to the residential living facility that is necessary.
