Provide assistance in patient care ADL’s / Activity and promote patient health including assisting patients with basic hygiene needs, collect patient data, such as vital signs, input/output and other delegated measurements in accordance with unit/facility policy and standards of care. Recognize and report abnormalities and/or change in the patients' health status. Promote continuity of care by accurately and completely communicating to other caregivers the status of patients.
