Responsible for performing psychosocial assessments, and obtaining 'Orders for Protective Custody' for those patients in mental health crisis, by evaluating patients psychosocial well being, their emotional and physical state.
Review liaison for private insurances, Medicare and Medicaid. Collaborate with health care team to facilitate proper discharge planning. Make appropriate referrals to community agencies that provide necessary treatment for patients upon discharge.
For those patients who need financial assistance, available resources located and ensured proper referrals are submitted.
