Member of the Hospitalist team responsible for delivery of Sound Physicians Core Processes designed to improve quality and efficiency in the care of hospitalized patients while promoting effective outcomes across the continuum of care. Current salary $105,000/yr.
- Partner with Hospitalist Chief in providing leadership and direction of the physicians, NP, and PA providers to implement metrics/measures and lead process optimization in delivering quality healthcare and lowering healthcare costs.
- "Deep dive" chart reviews to identify barriers for a timely discharge and more positive patient outcomes as well as looking for areas of opportunities to decrease readmissions.
- Facilitate coordination between the hospitalist team and hospital departments (nursing, quality improvement, case management, clinical documentation team) to ensure proper management of high-risk patients and form a plan for a successful discharge offering appropriate resources and services.
- Provide patient and family education to ensure understanding of the disease process, next site of care options, end of life discussions/planning, and community resources in line with value-based purchase.
- Strong, detailed oriented critical care clinical nursing experience.
- Participate in proactive evaluation of complex patients and initiate additional investigation, monitoring, and analyzing to be sure goals of care successful.
- Responsible for analyzing and evaluating data, measure key metrics, and track performance outcomes in relation to our At-Risk Scorecard as well as our internal metrics.
- Prepare and present information about the Hospitalists team's quality data to hospital administration as well as providing outcomes of Sound Physicians implemented processes.
- Proactively identify high cost diagnosis patients and arrange complex case meetings with multidisciplinary team to plan for best Next Site of care in the best interest of the patient.
- Provide remote assistance to 8 hospital sites in the East Coast and Mid-West as well as being onsite with the Hospitalist teams.
Total Joint Replacement Case Management - Western Maryland Health System

- Responsible for the overall operation of the Total Joint Program at Western Maryland Health System.
- Ensure exceptional customer service throughout the Orthopedic Continuum of care which includes pre-admission testing, peri-operative services, inpatient, and all other applicable entities with the orthopedic service line related to evidence-based for orthopedic patients.
