- Participate in the resident's interdisciplinary care planning process.
- Perform resident assessments of baseline, current medical condition, current disposition needs, and ongoing monitoring of residents' functional and medical needs.
- Conduct MDS reviews timely including face-to-face cognition and mood interviews with the residents.
- Document psychosocial needs, interaction and follow-up actions with residents and families according to facility policies and procedures.

- Facilitate documentation for resident's DME and home health or facility referrals
- Facilitate the discharge of residents when needed. This includes discussing options, providing resources, facilitating referrals and coordinating with community representatives.
- Ensure that the residents' therapeutic, nursing, and level of care needs are appropriately met by the discharge plan.
- Provide on-going psychosocial well-being support to the residents and families as needed. This may include visits to the residents, follow-up with psychiatry services, and follow-up or visits with the family.
- Provide quality service to all residents and guests
- Maintain awareness of surroundings, including observing residents within eyesight, being alert to their needs; summon assistance from Nurse Supervisor or other staff as needed
