About

As a goal-oriented, dedicated and experienced healthcare professional who has been working in healthcare for over eight years now. More so, I have worked as a community support worker for Open Skies Healthcare Organization. After a few months, my position changed to a treatment coordination, working with foster children who are under CYFD custody, I am experience and passionate with what I do as well as providing best care possible to children and their family, and foster parents and help them achieve their treatment goals.

Additionally, with strong interpersonal, mental health and effective communication skills, I easily connect with people and easily adapt in new environment as well. My problem-solving abilities, time management skills, organizing skills, attention to details, etc., allow me to address the diverse needs of the population I serve. I am also able to work effectively under pressure and with very little or without supervision.

Through my integrity, treatment coordinator, community support worker, and leadership qualities supervising TFPs and coordinating meetings like TTM, etc., with team members I am well-equipped to uphold the core values of your company. Coordinates and run all clients meetings smoothly like treatment team meeting together with the CYFD, client, foster parents, GAL, CASA worker, and other team members
Providing emotional and educational support to patients and families
Create goals for the client's treatment plan
Help to schedule client's doctor appointments for foster parent as well as attend other appointment like IEP meeting at client's school, as well to understand patient treatment needs
Develop a crisis safety plan with the client
Handle objections to proposed treatment
Present the treatment plan and consent to team members
Educate client(s) about their treatment plan, goals and objectives and ensure patients understand as well
Carry out home visit with foster parents and have sessions with client as well
Conduct home study for foster parent as well as supervision
