MLTSS/Medicaid Waiver Case Manager - Managed Health Care Company
Manage a caseload of 90 to 120 members with different ethnic backgrounds and socioeconomic status, covering 5 counties. Complete initial, annual, and change of condition assessments to set up and maintain care coordination and services for Medicaid Waiver members to ensure their safety and wellbeing while in their homes. Follow up from a hospitalization or nursing home stay to ensure they have a caregiver or informal support in place.
Work with provider agencies setting up home care services, home delivered meals, transportation and durable medical equipment. Complete phone calls and face to face visits on quarterly basis to reassess any barriers.
