- Vascular access management - Teach proper AV fistula/graft or catheter care. Reinforce infection-prevention and access-protection techniques. Assess for signs of infection, thrombosis, poor blood flow, or other access problems.
- Machine and treatment management - Educate patients on correct machine operation and troubleshooting. Review treatment prescriptions, treatment records, adequacy, and machine data. Reinforce proper preparation and storage of dialysis supplies.
- Patient safety - Teach recognition and management of emergencies such as hypotension, air entry, bleeding, access dislodgement, and other dialysis complications. Ensure the patient understands when to stop treatment and contact the dialysis team or emergency services.
- Ongoing patient support - Conduct follow-up visits/calls and reassess competency. Reinforce education and correct unsafe practices. Coordinate care with the nephrologist, dietitian, social worker, technician, and other members of the interdisciplinary team.
- Documentation - Document assessments, education, training progress, competency, interventions, and communication with the healthcare team. Maintain accurate treatment and patient records.
- Quality and regulatory responsibilities - Follow DaVita policies, infection-control standards, physician orders, and applicable dialysis regulations. Monitor quality indicators and participate in quality-improvement activities.
