As a Medical Case Manager, my primary objective is to support my assigned caseload, along with their 24hr support staff, with medical assessments, triage, and professional medical services when needed. I provide detailed reports of medical needs, treatments and outcomes to providers, loved ones and state agencies. I assist in developing and implementing treatment and behavioral plans to fit my client's needs. I provide many other ancillary support services to clients and staff as well.
