Gained case management experience working with a preceptor to achieve 120 intern hours.
- Discharge and Admission planning, ensuring insurance is active and covering services provided from the facility. Ensuring that, upon discharge, the patient has proper services if they are going home, the facility they are going to, and whether they have been approved.
- Initial mood, cognition, and Trauma assessments on new admits for the rehabilitation unit.
- Maintaining the accuracy of medications per diagnoses and diseases. Gross medication reductions are done for Long-term care residents to try to decrease certain medications, including psychotropic medications.
- Attending daily meetings and care conferences for residents to ensure residents are not declining and that medical staff can communicate any concerns or improvements.
Certified Behavior Technician - Centria Healthcare LLC

Provided ABA therapy for children with severe Autism and Behavior disorders, some of whom were non-verbal. Helped them achieve their independence through 5-15-minute timed-play trials and therapeutic instruction in daily living tasks. Taught behavior and coping skills, skills to help them play with others, while maintaining their independence.
- Ensured that individuals with severe autism and behaviors had the proper tools when they are out of control, and advice for parents and training for parents to ensure their children maintain positive lives with independence.
- Communicated with case manager, family, and supervisor about any improvements or declines in the child's therapy. When a child obtained a new skill, more were added to their plan. Most of these trials are based on things they may learn in school or their activities at home that need more assistance.
- Obtained QBS (Quality Behavior Specialist) training while working here.

- Twelve room halls, for one aide. Managed about 15 residents with minimal to no help from other aides as the residents were independent with mobility.
- Charted 100% accuracy in MatrixCare about the resident's mood, behavior, and activity level with daily living skills.
- Residents were mostly independent or needed minimal care and assistance with care.

Was a certified nursing assistant, then promoted to Restorative CNA, then promoted to CNA, Restorative, Central Supply, and Medical Records. Resident caseload fluctuated by job title. As an aide, there was a 12+/1 resident to aid load. Medical records were responsible for the entire building.
- Restorative: Assisted certain residents in Long-Term care with ROM exercises and therapy exercises.
- Medical Records/Central Supply: Input around 20+ residents' medical records daily, digitally through the EMR Point Click Care system by scanning documents on the scanner, then downloading them to the appropriate file and uploading them to the medical record.
- Ordered supplies, stocked supplies, and maintained the inventory of supplies Monthly/Weekly for the entire building, as well as managing the budget.

Received Employee of the Month while being here less than a year. Provided patient-centered care for residents in the memory care portion. Needed extra assistance with ADLs. Charted in the Point Click care system. Charted with 100% accuracy.
- Most residents were fully dependent on aides to help them dress, change, eat, bathe, etc, as they were unable to care for themselves due to dementia or Alzheimer's.
- Largest resident-to-aide load: 15 residents to one aide. Managed 15+ residents with minimal assistance from other aides unless it was in the patient chart that they were a two-assist.
