The My Patient Care Captain for the district which focuses on pharmacy managers, staff pharmacists, and technicians’ behaviors. Emphasis onoutreach calls, register prompts and schedule proficiencies according to the company’s guidelines. Align other teams,set high standards and expectations within their store. The Immunization Captain for the district which consists of weekly follow-up with district leaders and captains from other districts in the region. Provide each store in the district with the resources they need to be successful which includes a weekly immunization tracker, stock trackers, and bag tags that were made specifically for this purpose. Monitor progress and reiterate expectations during unannounced store visits. Inspire and excite other teams to work together towards a common goal for the district.
Focus on root cause analysis of two stores in the district that are underperforming or not performing at their highest potential. Observe colleague behaviors, performance, provide examples, resources, and action plans for improvement. Work alongside struggling pharmacists who need direction to be successful. Follow up monthly with the regional manager on progress.
Perform pharmacist duties such as drug therapy review, drug interventions and medication management. Follow-up with patients on new fills, adherence issues, answer any of their questions/concerns, and overcome common barriers for patients such as cost of therapy and unwanted side effects. Recommend add-on therapies, removal of duplicate therapies, and lower cost alternatives. Provide clinical expertise regarding usage of medication, interactions, side effects, and over-the-counter products to ensure optimal therapy. Follow-up with insurance companies on audits, prior authorizations, and claims. Develop strong relations with customers and provide support to enhance patient experience.
