About

As a Nurse Case Manager for the past 8 years- from 2015 for Davita Kidney Care in the Phoenix/Tucson ESCO market until January of 2019 and currently for United Healthcare Community Transitions Program. In these positions, I am responsible for: Evidence-based clinical guidelines/management plan, discharge planning and assistance with patients and their families, identification and clinical management of risk factors and comorbid conditions, identifying and addressing fragmentation of care and filling the gaps, coordination of care among PCPs, case managers, nurses, nephrologists, and other specialists, utilization management, patient health education to promote self-management of their conditions, and quality-of-life improvements for all my patients. I am passionate about caring for people and have found great pleasure in my ability to improve the services throughout my career.

Advocacy, mentorship, multi-tasking, attention to detail, and collaboration are qualities that come naturally to me and find that it is important to advocate for realistic changes that not only meet the needs of the patients I serve, but also meet the needs of the teammates around me. I do not hesitate to be the voice that encourages change and makes a positive impact. Mentorship is critical because trusting and having an authentic partnership with team members and patients can dramatically improve progress towards achieving goals.

Collaborating with other programs and service providers is critical in meeting the needs of the patient and can be used to connect a patient to useful resources and ease them through the process of receiving support.
