• Monitoring that patients collect their chronic medication and do not miss their dates for collection, drawing of blood samples to ensure that all is in order, capturing of stats and data to reconcile the change in Months of patients' attendance.
• Facilitate HIV status disclosure to partners/families on ART.
• Support and facilitate recruitment of index case contacts (partners/children) for HIV testing services.
• Facilitate enhanced adherence counselling, and support tracking and linkage to care, for all patients who have missed their appointments.
• Update patient files and provide these to the data capturer for entry into TIER.Net and the Demonstration Project database.
• Forge links between the HIV/TB programme and other programmers/services, e.g. Department of Social Development; non-governmental and community-based organisations.
