• Make phone calls arrange home visit and work with community
• Make notes and regularly update the appropriate Peer mentor and return tools when they contacted the client
• Support the clinic teams to trace, contact and bring to care those clients that are urgently required to return to the clinic
• Document peer mentor and navigation data from clients encounters
