Manage a complex caseload of children, young people and adults with learning disabilities, autism, ADHD and co-occurring mental and physical health conditions.
- Act as Care Coordinator/Key Worker, delivering person-centred, evidence-based and recovery-focused care.
- Undertake comprehensive biopsychosocial, neurodevelopmental and mental health assessments, including Mental State Examinations (MSE), clinical risk assessments and safeguarding assessments.
- Undertake comprehensive Adult and Children ADHD assessments using structured, evidence-based assessment frameworks in accordance with NICE NG87 and DSM-5 criteria. - Utilise recognised ADHD assessment tools, including DIVA-5, ACE+, ASRS and other structured clinical interviews where appropriate.
- Complete ADHD and Autism referrals and support individuals throughout neurodevelopmental assessment pathways.
- Carry out clinical triage, prioritise referrals and coordinate timely assessment and intervention.
- Develop, implement and review person-centred care plans, Positive Behaviour Support (PBS) plans, crisis plans and risk management plans.
- Participate in Dynamic Support Register (DSR) meetings and Care (Education) and Treatment Reviews (C(E)TRs) to support admission avoidance and improve outcomes for individuals with complex needs.
- Work collaboratively with the Integrated Care Board (ICB), psychiatrists, psychologists, occupational therapists, speech and language therapists, GPs, social workers, education providers and voluntary organisations to deliver integrated care.
- Complete Mental Capacity Act (MCA) assessments, contribute to Best Interest meetings and safeguarding enquiries where required.
- Provide specialist advice, education and support to individuals, families, carers and professionals regarding learning disabilities, autism, ADHD and mental health.
- Maintain accurate clinical documentation and contribute to clinical governance, audit, service development and continuous quality improvement in line with NMC standards and NICE guidance.
